CITIZENS POLICE ACADEMY APPLICATION
Due to the sensitive nature of the information required on this application we apologize for being unable to offer it as an online submission, but if you'd like to attend our next Citizens Police Academy you can download this .pdf application, complete the form and return it to our Public Relations Office. A background check will be conducted to insure that you are a good candidate for the free 12-week course where you will get an up close and personal look at real police work like you've never seen it before. CITIZEN REPORTED ACCIDENT FORM
This form is designed to enable citizens to report motor vehicle accidents that are not investigated by police. Use this form to report an accident that occured on private property (such as a parking lot, for example) or when circumstances make an immediate police investigation of a roadway accident impractical or impossible (such as ice, impassable snow, natural disasters, etc.). This .pdf form is available for download only. Turn the completed form in to your insurance company. Note: This form may only be used to report a non-injury motor vehicle that happened inside the city limits of Hot Springs, Arkansas.
STATIONARY RADAR/TRAFFIC ENFORCEMENT REQUEST
This interactive online submission form enables you to make a request for the placement of our stationary radar unit in an effort to encourage drivers to reduce their speed. You may also use this form to request enhanced traffic enforcement support for areas that are experiencing an identifieable problem such as traffic congestion st specific times, recurring reckless drivers, problems associated with ongoing construction, etc. Complete the form and submit it online and a member of our traffic section will investigate the issue further.

As a courtesy to the public the Hot Springs Police Department will, at your request and as time and manpower permits, increase patrols in the area of your home or property when you plan to be away for an extended period. Our online House Watch Request form is an interactive submission form that you can fill out online and submit to us electronically. Your request should be made at least 5 business days prior to your scheduled departure and it may remain in effect for up to 14 calendar days. Please read the instructions carefully and provide as much detailed information as necessary.

In an effort to help educate the public we offer a ride-along experience to selected applicants who meet the minimum application guidelines and approval process. Generally, this opportunity is extended to those who attend our Citizens Police Academy. To apply for a ride-along experience you can download our application, complete the form and return it to our Public Relations office. A background check and an interview will be conducted to insure that you are a suitable candidate for a police ride-along experience.

STOLEN PROPERTY LIST FORM
The Stolen Property List Form is provided for you in the event that you are the victim of a theft, burglary or breaking/entering where you have multiple items missing. AFTER YOU HAVE MADE A POLICE REPORT you can download this list and do a thorough inventory making note of your missing items, their descriptions, serial numbers, etc. Once your list is complete you will need to turn the list in to our Criminal Investigations Division so it can be included as a supplement to your initial police report. This form may also be used to record any items that you discover missing later. This form is a .pdf for download only and is not an online submission form.
If you have a theft, burglary or B&E, call the police first - before you start taking inventory - in case there is evidence like fingerprints, etc. at the scene that needs to be collected.
GAS DRIVE-OFF COMPLAINT FORM
Failure or refusal to pay for gas at a retail pump, for whatever reason, is considered THEFT. The Gas Drive-Off Complaint Form is designed exclusively for retail stores that sell fuel to use to report those incidents. If a customer fails or refuses to pay for fuel that he/she pumped into a vehicle at a retail gas pump and has already left the business without using force to commit or escape from the crime the store can download this form and use it to report that theft. Once the form is completed the store may call the police department's non-emergency number at 501-321-6789 and we will send an officer to pick up the report.
This form is not an alternative to a police reponse when a crime is in progress. If a customer who fails or refuses to pay for gas is still on the scene or the customer threatens to use, or actually uses force to commit the crime or escape from it, call 911 immmediately.
SHOPLIFTING COMPLAINT FORM
Failure or refusal to pay for retail merchandise, for whatever reason, is considered THEFT. The Shoplifting Complaint Form is designed exclusively for retail stores use to report those incidents. If a customer who fails or refuses to pay for merchandise at a retail store and has already left the business without using force to commit or escape from the crime the business can download this form and report that theft. Once the form is completed the store may call the police department's non-emergency number at 501-321-6789 and we will send an officer to pick up the report.
This form is not an alternative to a police reponse when a crime is in progress. If a customer who fails or refuses to pay for merchandise is still on the scene or the customer threatens to use, or actually uses force to commit the crime or to escape from it, call 911 immmediately.
SELF-ASSIST REPORT TOOL
The tool is designed to allow our citizens to self-report minor incidents that occur within the city limits of Hot Springs. This report is NOT designed to report incidents that are in progress nor is it designed to replace calling 911 in an emergency. Minor incidents that may be reported include, but are not limited to: misdemeanor thefts, online harassment, criminal mischief, fraud, suspected drug crimes, etc. Please call 9-1-1 in the case of an emergency.
This form is not an alternative to a police reponse when a crime is in progress. If a customer who fails or refuses to pay for merchandise is still on the scene or the customer threatens to use, or actually uses force to commit the crime or to escape from it, call 911 immmediately.
